You start lagering after you reach your final gravity. Fermentation should be complete (or nearly so) while the beer is still in primary. During the D rest (assuming you do one) the yeast are not really fermenting, they are cleaning up by products produced during the active fermentation stage. This continues at a much lower pace during lagering.
If you start lagering before fermentation has stopped, your attenuation will stop a little short of where it should be.
