Well, back sometime around 96, I brewed a Bo Pils (extract, at the time). It was cold as fuck in the garage, and I was having trouble maintaining a nice boil. So.... I left the cover mostly on (who knew?). Well, I tasted it after primary and it tasted like olives and cabbage fucked and had mutant cabbageolive kids in my fermenter. Down the drain it went.

I got me a mesh bag, some oak chips etc. The book said to weigh it down with some stainless washers or something. Found some, dropped the whole thing in there. Month or so later came to sample. Tasted like liquid steel. I guess either the washers weren't stainless or they had something on them that I didn't clean off; it was truly awful, and maybe even toxic. Sad day.
