When was the last time you calibrated your thermometers? If you're falling outside the optimum saccharification bracket it will reduce your extract efficiency.

Although grain crush is the likely culprit, brewing super low gravity or high gravity beer can also greatly effect your efficiencies. I'm talking about under 1.040 or over 1.080 beers.
