Up until the last few brews I was only adding the amount of water that I needed and drained the MLT completely and was still having this problem.

Like I said I drain directly to the bottom of the kettle and the wort gets what looks to be some pretty good mixing action filling from the botom up.
Even when I have pulled off equal amounts of wort for the boil I still have a fluccuating effeciency.

All of you guys get the same number every brew day?

Post boil 5.25 gallon @ 1058 shooting for 1066 WTF??

Unpossible. Sugar doesn't disappear once it is in there. Volume can hide, though.

If preboil volume and gravity are accurate, you have 377 points of gravity. If postboil gravity is accurate, divide that 377 by 66 (from the 1.066) and you really had 5.7 gallons post-boil.

I'm gonna take a wild guess and say that the 5.25 gallons you reported were in the fermenter and you might have left 1/2 gallon behind in the kettle mixed with the trub and hops and stuff? Perhaps accounting for kettle losses will make things more consistent?

Maybe a quart of kettle loss or less, but I have no idea how much the hops are absorbing. 4oz of whole leak in the last brew.

Whole leaf absorb about 3/4gal per pound, so at 4oz, that's at least 3/4 quart.
